The role of the Community Social Worker is a dedicated professional who works with individuals facing various challenges in the community such as mental health, disabilities, or social isolation.
The Community Social Worker connects people in need to various services and provides short-term case management to empower clients to live a life of stability.
Responsibilities for Community Social Worker Print schedule for front desk to assist with check-in process.
Adapt to new technology software to improve efficiency of department.
Empower clients through strength-based goal setting to lead independent lives.
Assess client's needs, strengths, and challenges to develop personalized support plans.
Advocate for client's rights, ensuring they have access to needed services.
Assist clients in acquiring and enhancing life skills through a variety of programs including budgeting.
Collaborate with all EASS staff and outside agencies to ensure comprehensive care plan for clients.
Educate clients and community on available services, life skills, and health related topics.
Adheres to high level confidentiality for staff and client information.
Keeps accurate up to date case notes on client's progress.
Participates in weekly case consults meeting with Executive Director and Resource Coordinators.
Participates in weekly staff meetings with all departments of EASS.
Resolves staff and client conflict in a peaceful manner.
Complete client follow-ups at 30-day, 90 month, 6 months, and 12 months.
Collects monthly statistics and work with Executive Director to review data for KPI All other duties as assigned.
Qualifications for Community Social Worker Bachelor's or master's degree in social work 3
years of social work, nonprofit, or human service experience Excellent listening and communication skills Ability to make psychological assessments and develop care plans.
Ability to work extended hours if needed.
Ability to use translations technology services.
